If you are a parent, and would like to start the process of seeking help for your child, review the "Make An Appointment" link above.

To receive the enrolment packet prior to appointment (which contains questionaires, contact information and other required documentation), you may put in a request on the Online request form or call us at +65 6466 8090 for these to be sent to you.

For us to be able to retrieve your request online, please remember to include your Email Address.

If you live outside of Singapore, you may request for an appointment online and we will get back to you within 48 hours (on working days). Alternatively, for urgent appointments, please call our Case Managers or Clinical Coordinators at +65 6466 8090 and we will see what we can do for you.
