Referral InformationTo help maintain consistent standard of care for new patients at Ovspring, general procedures have been outlined for referral for paediatric and/or allied health services as follows: Referrals from Primary Care Physicians & Healthcare Professionals 1. Referrals are accepted from primary care practitioners and/or other healthcare professionals. 2. Clinicians are requested to fill out the Healthcare Professional Referral form, which can be downloaded in PDF format at the download section. 3. Upon completion of the referral form, please fax this to +65 64671344. 4. Our Clinical Coordinator (Paediatrics) or Case Manager will call you within 48 hours to schedule an appointment or request for more information should this be needed. 5. For acute or urgent cases, please call our Appointments Line at +65 64668090 to speak to our Case Manager or Clinical Coordinator (Paediatrics). Self Referrals from Parents & Others 1. Self referrals from parents (or guardian) are also accepted. If your child has not had an evaluation before, an initial or intake consultation may be recommended to help determine what your child's and family's needs are prior to assessment. This will help parents, guardians or caregivers understand the assessment process and how best to help the child. 2. If you are a teacher or education professional, and feel that the child you are working has learning or behavioural difficulties, you should encourage the child's parent to make a referral. Should teachers require information concerning assessments or process of diagnostic assessments at the Clinic, please contact us via our information request form. 3.
