Intake & Initial Consultation

Do you have a child that seems to be having difficulty with age appropriate activities? Do you feel your child may be delayed in one or more areas of development? Are there challenging behaviours that are of concern?

An intake or inital consultation is recommended when a family doesn’t know where to begin. An initial consultation is a 60 to 90 minute consultation with our Paediatrician, one of our Professionals or if requested, with a team of our Professionals.

To being this process, we will require you to provide us with information about your child. This information is first obtained through parent questionaires that you will need to fill out. Depending on the issues of concern, the child's teachers may also be requested to complete teacher questionaires.

This process helps us obtain a comprehensive picture of the child or adolescent, their difficulities as well as their abilities in different settings. It is important therefore that questionaires be filled as fully as possible. At this time, we will also request for all relevant recent reports, test results, IEP (if any) or any other information that will help us understand your child and his/her abilities. Upon receipt of your completed forms and reports, we will review all information on hand.

After reviewing the information on hand, our office will contact you to schedule an Initial Consultation. This could either, be with a Specialist or with a team of Specialists, depending on the child's needs and situation. Our team works from an inter-disciplinary perspective, towards meeting the needs of the whole child and your family.
